Self- Reflection

Geriatric medicine, varied from the previous rotations I have completed. It focused more on the chronic conditions and the multitude of conditions related to the patient. Managing the different conditions efficiently were an important aspect of geriatric medicine. This leads into poly-pharmacy and how unnecessary medicine should be avoided. It is important to note that many patients are not mentally capable of understanding their symptoms. One of the patients I saw at this rotation had severe traumatic brain injury as a teenager. During my site evaluation I presented, 2 full length H&Ps and 10 pharmacology cards. My site evaluator was Professor Davidson. Throughout, this site evaluation I learned how it is important to make my own clinical judgement as well rather than just trusting the provider. It is imperative to be critical about the decisions being made related to the patients. Critical analysis of each plan for the diagnosis is important as these patients tend to be facilities for a prolonged period of time. Hence, new findings can be easily missed. Journal Article & Summary

The articles name is Music-based therapeutic interventions for people with dementia and was published in 2018. This study included RCTS that compared music based therapeutic interventions for people with dementia. Control groups received no music therapy. In total 890 patients were included in the meta-analysis. It was concluded that music therapy improves emotional well-being and quality of life. Anxiety and depressive symptoms were reduced. However, further research should be conducted on the long term impact of music therapy.

Reflection

Ambulatory Medicine rotation involves a wide range of conditions and provides exposure to a variety of procedures. The treatment strategy in urgent care differs from the previous rotations I have done. The strategy used here is to figure out if the patient’s concerns can be handled as outpatient or requires care at a hospital. Although sometimes this can be redundant there were also several moments where the patient’s concerns differ.  At this urgent care I was able to perform several different procedures which included suturing, IM injections, venipucnture, ear irrigation and splinting. This study compared patients that received the topical ciprofloxacin 0.3%/dexamethasone 0.1% otic suspension with those  that received polymyxin B/neomycin/ hydrocortisone otic suspension for the treatment of acute otitis externa .The goal of the study was to compare the clinical outcome of these patients. Site Visit Summary: My site evaluator was Professor Rachwalski. Discussing the H&Ps with him helped improve them throughout the rotation and provided a different approach for treating patients. Starting from the chief complaint, creating a broad range of differentials is important. In the physical exam portion, documentation should be done in the way a physical exam is done. This will help prevent any documentation errors and ensure a full work up was done. Rather than stating this is the likely diagnosis, it is imperative to be confident in the diagnosis being given.
