Site Evaluation Presentation Summary 

My site evaluator was Professor Rachwalski. For the first evaluation I presented one full H&P and 10 pharmacy cards. My H &P was about a patient who had a pulmonary embolism secondary to a recent COVID-19 infection. We discussed changes that need to be made in the HPI. The HPI I wrote did not consist of all the necessary information. We discussed a  mnemonic-  OLDCARTS to ensure all the pertinent information is in the HPI. It is also important to ensure all the patient’s medical history is repeated in the PMHx section even if it is listed in the HPI. For assessment and plan it is necessary to be concise and direct about the patients presentation and diagnosis.  I incorporated these lessons for my second evaluation where I presented 2 full H&Ps and a journal article.  The HPIs included all the necessary information and the assessment and plans were direct. As I start new rotations, I will ensure I implement these techniques to my H&P writing skills.
