Site Visit Summary:

My site evaluator was Professor Rachwalski. Discussing the H&Ps with him helped improve them throughout the rotation and provided a different approach for treating patients. Starting from the chief complaint, creating a broad range of differentials is important. In the physical exam portion, documentation should be done in the way a physical exam is done. This will help prevent any documentation errors and ensure a full work up was done. Rather than stating this is the likely diagnosis, it is imperative to be confident in the diagnosis being given. Reasons should be explicitly stated and then diagnosis should be provided.
