Self Reflection

My internal medicine rotation was at NSUSH. This rotation was a phenomenal experience where I was not only able to learn medicine but also do procedures.I was given the opportunity to do procedures such as  venipunctures, ABGs, insert foleys,  chest tubes and shadow TAVRs, colonoscopies and endoscopies.  I rotated in MICU for one week which was a very humbling experience. It was interesting to see how almost all aspect of the human body needed to be under observation while being admitted.   An interpersonal challenge I faced during this rotation was how death is also a huge part of medicine. Site Evaluation Presentation Summary 

My site evaluator was Professor Rachwalski. For the first evaluation I presented one full H&P and 10 pharmacy cards. My H &P was about a patient who had a pulmonary embolism secondary to a recent COVID-19 infection. We discussed changes that need to be made in the HPI. The HPI I wrote did not consist of all the necessary information. We discussed a  mnemonic-  OLDCARTS to ensure all the pertinent information is in the HPI. Summary: In order to relieve esophageal impaction, glucagon is frequently used. This study was a systematic and meta-analysis that evaluated the efficacy of glucagon for esophageal impactions. The studies that were included were  either retrospective, prospective observational, and randomized controlled trials. These studies had either a control group or placebo and must have been conducted on humans. Five  studies were selected and ages of the patients ranged from 5.1 to 59.5 years of age. Site Evaluation Presentation Summary

For my site evaluation I completed three full H&Ps, 5 drug cards and a journal article summary. I presented two H&Ps to my site evaluator. The journal article summary was related to depression and physical activity. The 5 drugs I researched were Escitalopram, Benzodiazepine, Aripiprazole, Haldol and Risperidone. For my first evaluation the H&P I presented was not as organized. Rather than focusing on the pertinent information and positives, I presented the full H&P. Journal Article

The purpose of the article was to evaluate the relationship between physical activity and depression. It was a meta- analysis that included 49 prospective studies from Asia, Europe, Europe, North America and Oceania.   These studies compared people with low vs high levels of activity. It was found that higher levels of activity was associated with lower odds of developing depression. Physical activity also has a protective effect on the emergence of depression in youth and elderly.
